Professional Leave Procedure & Forms
- Attending professional development that includes expenses? Complete the Professional Meeting Request from linked above.
- This form needs to be signed by your building principal and then sent to the district office for Dr. Cripliver's signature.

Once it has been approved by both the principal and superintendent, your professional leave request will be placed on the agenda for the next scheduled board meeting for board approval.

It is important that this form is filled out immediately so that it can be approved in advance of the meeting, workshop, date, etc. when requesting reimbursement of expenses.

Once the board has approved the leave, you will receive a letter from the superintendent's secretary with the forms you will need to fill out requesting reimbursement for your expenses. (Registration, mileage, meals, hotel, etc.)

You are responsible for registering for the event, as well as making hotel reservations if needed.

All expenses are your responsibility.

You will be reimbursed after the next scheduled board meeting provided you have completed the proper paperwork and submitted it to the superintendent's office prior to the next board meeting.

Forms must be accompanied by itemized receipts for food and hotel. The corporation will only reimburse for hotel room and tax, registration, mileage, and meals (excluding alcohol).
- You must also fill out the Pre-Arranged Leave Day Form the week before the leave in order for us to obtain a substitute for you. Please do not send it any earlier in case of cancellations or changes to your plans. Fax the Pre-Arranged Absence Form to Pam Porter. (Fax: 962-4011)

A professional assignment may be requested to attend a conference, meeting, or workshop if no request for reimbursement is made.

Approval for a professional assignment may be granted by the principal and superintendent.

Completion of the Professional Leave Request Form and the Pre-Arranged Leave Day Form, once the professional assignment is approved, needs to be filled out and submitted a week prior to the conference, meeting, or workshop.
